Initial Claims
If you have not had a hearing yet with a judge, please
fill out one of our Social Security case evaluation
forms:

Short
form - we estimate this will take approximately 5 minutes to fill
out. But it may require that we get additional information from you to
fully evaluate your case.

Long form
- we estimate this will take you 10-15 minutes to fill out, and it will
give us the most information about your case.
Appeals (Appeals Council & Federal Court)
If you had a hearing with a judge and received an
unfavorable decision, you will need to
appeal to the
Appeals Council.
If you received a denial from the Appeals Council, you
will need to file a complaint in Federal Court.
